St. Paul School
Parents,

Welcome!  Current health information is required for all new incoming students, including transfer and pre-kindergarten students. In order to plan for health care at St. Paul School, we ask your cooperation in completing and returning the required health forms.

As the school nurses, we want to inform you of our policies regarding immunizations and health records for all students.  All of our policies regarding immunizations are in compliance with the school health regulations put forth by the Massachusetts Department of Public Health. These policies are required to be put in place for both public and private schools. In addition, St. Paul School policy allows for medical exemptions when a physician has documented that an immunization would be contraindicated for a child. Our policy does not allow for religious or philosophical exemptions.

Please be aware that St. Paul School is completely NUT FREE!   Nuts of any kind are NOT allowed at St. Paul School.  Please be considerate of others and follow this strictly enforced policy.  Thank you for your understanding.

For all NEW students (including Pre- Kindergarten and transfer students)
All new students must provide a copy of their immunizations and report of physical examination for their health care provider for school entry. By public health law, the examination must be dated within a year of starting school. Please note that ALL immunizations must be up to date and that evidence of a negative tuberculosis test in the past or statement of low risk tuberculosis must be included on the physical exam. Also, if your child has had a natural case of the chicken pox, please make sure that this is written on their record.

Please make sure that ALL records are sent to St. Paul School by Friday, September 2nd!
If your child is transferring from another school please request that their health records from their previous school is sent to St. Paul School as soon as possible!

If your child is entering Pre-Kindergarten or Kindergarten
Please provide us a copy of a physical exam completed within the past 12 months-including documentation of a lead test and immunization records. 

Students entering Kindergarten are required to have the following immunizations:
       3 doses of Hep B vaccine
       
5 doses of DtaP/DTP
      
4 doses of Polio
       
2 doses of MMR
       2
doses of Varicella vaccine

If your child is entering Grade 4
         Please provide us a copy of a physician exam completed within the past 12 months

If your child is entering grade 7

         Please provide us a copy of a physician exam completed within the past 12 months.

Students entering grade 7 are required to have the following immunizations
(In addition to those required for kindergarten):
        
1 Booster of Tdap
         2 doses of MMR vaccine
        
2 doses of Varicella if given after the age of 13 OR physician documented case of chicken pox including date

Medications:

No student is to carry medication with them in school including nonprescription medications. The only exceptions to this rule are those students requiring emergency medications: inhalers for asthma, and EpiPens for allergy.
Students must have a written order from their physician and permission from parents to have medications in school (including emergency medications and nonprescription medications). Please note: All medication sent to school to be given to your child by the school nurse must be a properly labeled pharmacy or manufacturer’s container. If you require a container for school and home, please ask your pharmacist to give you two labeled containers.

New Students or Transfer Students with EpiPens:
Parents of new students or transfer students who require or have been prescribed an EpiPen should contact the school nurse by telephone prior to the start of school to inform the school nurse of the student’s allergies. At that time we will also send out the necessary forms to your home address.

Parents must provide their students EpiPen and also provide a written order from the prescribing doctor and the parents signed permission to administer the EpiPen, if necessary. Parents also need to provide a backup EpiPen for the nurse’s office. Each child should have two EpiPens at school- 1 for the classroom and 1 to be kept in the nurse’s office.

Health Needs:
If your child has a health issue to be addressed (severe allergies, broken bones, nosebleeds) we ask that you contact the school nurse directly by telephone as this allows time to plan for their care.
